Sports Physical: A sports physical is required to try out/participate in any interscholastic activity. Sports physicals are good for 12 months. If you know your child is interested in any of these activities, please scan and email his or her sports physical to the following email address: healthforms@d181.org. Please put your student's full name and date of birth in the subject line. You do not need to wait until the season is upon us. The sooner you get the physical turned in to our nurse, the easier the process will be.
